Attached are two versions of the same shift plot for a production of Hello, Dolly.  The one organized "by scene" was the original worksheet we created during paper tech to make sure everything that needed to be shifted was included. 

Then the data was re-sorted, this time "by crew," and some of the columns were swapped, in order to create a version which could be posted for quick reference backstage.  We also cut this version apart and gave each crew member a single slip of paper listing all of his/her shift responsibilities.

Excel format.

This is my 'work horse' scene shift paperwork good for record keeping and for running crew to follow during a show.

I was the ASM, sorry my track is highlighted throughout.

Also, I should mention that these shrink very well to 5x7 cards for individuals which I have formatted on my computer if anyone is interested.

Deck Shift

This is the PDF file of the deck shift paperwork I created and used as an ASM during my school's production of South Pacific this past spring; this is the paperwork I gave to my crew to run off of, and it is what I used to run and follow the show backstage. I think because of the fact that I use Excel 2013, files save as .xlsx files, so I couldn't post a working/editable document. I am willing to email the working file to anyone who would like to use it though.

Here is the scene shift list for the production for Marry Poppins we just completed a few weeks ago.  For some clarification on the set pieces, 1-3 were huge multi-sided platforms & 4-6 were small multi purpose platforms. Red text = flying people and blue text = "magic" aka a lot if invisible string. :)
