Mike, concerning typed scripts: if everyone is not using the typed version, I will mark up mine with the page breaks in the original script and use those numbers as the page references on everything. 

more on topic, I still do all my books by hand, but clearly need to give the Word option a try!

I wasn't on production staff for this, but the theater I worked for in Ohio once held curtain on a production of ALL MY SONS directed by Rebecca Miller for almost an hour and a half cause her dad's plane was delayed.  But I guess if you're gonna hold for someone's dad, Arthur Miller is a good choice.  Luckily, she was there and did an audience Q & A to fill the time.

I've used a tarp a number of times (mostly large canvas paint drop cloths) and yes slipping can be a problem.  But if you can invest in a few of those carpet gripping mats (that go under carpets) it helps A LOT.  Between that and strong gaffe tape on the corners to hold it to the floor, I've been ok.  I still wouldn't try to dance or fight on it, but just normal walking is fine.

another option for rehearsals that move is to get a large tarp (you can use canvas paint drop cloths) and tape out the set on the tarp, which can travel from room to room with you.  Be sure to tape all the corners and most of the edges well to prevent wrinkles and know that it isn't the safest option if you have a lot of fights or dancing.

Another key is to establish early on the terms you will use for blocking - i.e. establish a north, south, east and west so that everyone is on the same page with where they are moving.  And watch to make sure there is a crossover from one side to the other - sound basic, but can wind up being a huge pain! And the advice of having people watch from both sides (and both ends of each side) is an important one.  Treat it as being in the round.

I always time the first read just to get a sense of where we are starting.  Then I usually don't pick it up again until we are running chunks.  Unless, like Matthew said, there is something specific, like a scene change or costume quick change that needs to be timed all the way along.

Another option is to contact the American Symphony Orchestra League - the professional association for orchestras.  They may be able to point you in the right direction.

If my page numbers don't match the original script when I am done, I go through and mark my pages with the original script's page breaks and page numbers so that I can use them when referring to things with actors (or anyone else not using my version).

I've had several instances where something has happened to an actor when no understudies were available.  Once due to an injury received during a performance, once for the death of a parent the morning of a matinee, and once for extreme illness on the part of the actor.  In all three cases, these were small non-equity companies that do not regularly hire understudies and each time the performances were canceled.  In each case I, as SM, was asked my opinion, but the decision was made by the management of the theaters involved.
