Stage Management: Other / Welcome packet

I am stage managing a Chinese Opera this summer and my director has asked me to make welcome packets for the 8 stars that we are bringing from China.  (Rehearsals and performances in NYC)

I am trying to figure out what information to include.  I want to keep it simple, seeing as how everything needs to be translated into Mandarin Chinese. 

I got subway maps.
I will include addresses of hotel, studios, theatre, etc.
I will scope out Chinese restaurants near these places and grab menus.

Luckily don't have to deal with parking or anything like that.

But what else should I include?  Any suggestions?

The Hardline / AEA Quarterly Insurance Premiums

Make sure to pay your quarterly insurance premium if you are eligible for health insurance through AEA.

AEA is asking that you remind your cast members as well.

"Since changes went into effect in our Health Plan requiring the payment of a $100 per quarter premium by Plan participants, a number of members have missed the deadline for paying the premium.  In many cases this was because the member was working out of town and didn't receive the bill.

 Equity is [providing] a flyer that should be posted on your call board as a reminder to your company of when the premium payments are due.  Please be on the lookout for this envelope that will come to you at the theatre address where you are currently working. We would also like you to make the following announcement at half-hour: "I've been asked to give you a reminder that the next Equity-League $100 Health Insurance premium is due on June 1st.  If you think you are eligible to start or continue coverage as of July 1st, but have not received a bill, please contact the Equity-League Health Trust Fund office."

 Although the next premium deadline is June 1, and you probably won't get the flyer until that date or after, there is a grace period of a couple of weeks.  If anyone in your company thinks he/she should have received a bill but hasn't, please make sure they contact the Fund office as soon as possible at 212-869-9380 or 800-344-5220.
 
 Thank you very much for your cooperation on this.  In the past two quarters, too many members have missed out on coverage because they did not pay the premium on time.

I am wondering what do you do on your paperwork (rehearsal calls, sign in sheets, dressing room signs, etc).  Do you use the first and last names together or last name with Mr./ Ms. or just first names?

How have you found the cast reacting to or having an opinion to this?

I ask because I traditionally use both first and last names.  The SM on my current project (I am ASM) prefers Mr/Ms Last Name.

I am of the opinion that outside of opera and a few dance companies, everyone goes by first name without question, therefore using first name with last name on paperwork is less confusing for others to read.  

I found this to be particularly true on this production when I made dressing room signs and mirror station signs with Mr/Ms Last Name and came into the dressing room and the actors had written their first names on the signs.

When does respectful become stuffy or just plain annoying to performers?

The Hardline / Showcase Agreement

I am looking for a copy of the AEA Showcase Code Agreement and I cannot find it amoung the other rulebooks on the AEA website.

Does anyone know where I can find a copy?  Or who the showcase rep at AEA is?
