After trial and error, I now don't keep my 'in-use' paperwork in my script folder at all. Once we're into the run I keep copies of running paperwork (fly plot, lx cue list, ASM plots) etc in the book, but during rehearsal I keep everything in a display book folder like this https://www.amazon.co.uk/Premium-Quality-Pocket-Display-Folder/dp/B002LFL006.

I've found it's much easier to have everything in plastic pockets that you can put flat on the desk and discuss with the director or whoever, and I'm not flipping back and forth in the script folder, losing my place in the scene. I buy them in bright neon colours so they're less easy to mislay.

As for order, set it up how you think, then change it when you realise you keep looking for something in a place where it is not, and so it needs to be put there!

I work in UK opera & I do calls exactly how I do in theatre, which is how most people do them here. We only do a 5 minute during the show & it goes "You call please Miss A & Mr B, Miss A & Mr B your call, thank you".

The same for me, I also work in UK theatre and opera. It's an accepted thing that 'your call please' is 5 minutes, and "Mr x to the stage please, Mr x to the stage", is 'Where the hell are you, your entrance is in 32 bars'

Out of curiousity, does US Equity permit two members with the same name? UK Equity doesn't, even if they are an SM and an actor.

When I joined UK Equity (admittedly mostly for the public liability insurance cover they offer as a benefit - the differentiation between union and non-union gigs here is not so strongly delineated as in the US), there was already an actor member registered under my legal name. So I had to change my name to join, I chose something very close to my original name. I'm not married, but should I marry in the future, the thought of having a third name is just too much!

Coming late to this thread, but there are two performances remaining of 'Phone Home' which is performing simultaneously in London, Munich and Athens, at 7pm British Summer Time tonight, 7pm GMT tomorrow (the hour changes tonight in the UK and Europe). Not sure if this will be accessible to anyone outside of the UK, it's on the BBC iPlayer. The documentary is mostly about the volunteer performers, but also looks at the technical and creative aspects. We get to hear the showcaller cueing the Red Arrows, James Bond and HM The Queen!

Talking of page mic embarassment, on the panto I did this Christmas, the CSM took great delight in trying to make me giggle when I was calling Beginners. Disclaimer : we were at least 25 shows into a 40+ show run, the cast knew their calls, it was by no means every performance, and I always managed to do the full call. It just sometimes took 2 or 3 attempts  :) Luckily cast and crew all thought it was hilarious.

The other option, if you are worried about confusion, would be to put the page in multiple times.  Using that PDF you linked as an example, you would copy the page into your book twice.  On the first page cross out (or cover with blank sheet when you are at the photocopier) the extra bars after the repeat section and indicate a page turn.  Then at the top of the second page just indicate in a way that is clear for you that you are now following the lyrics for the repeat and when you hit the end of that section you are good to go. 

This is what I do if in doubt. For one song with a simple repeat, I mark the cues as 'First verse only' or whatever, but I'm currently working on a baroque opera which has repeats in virtually every aria (generally ABA, ie sing right through, go back to the beginning and sing again to such and such a point), and I've separated out and recopied all the repeats. It means there are a few odd half-pages  of music, but it works for me. I mark page numbers as 73, 74, 75, 76, 77 [half page] , 73 repeat, 74 repeat, 75 repeat, 77 after repeat [half page] etc

Judging by the OPs username, they are from the right-hand side of the pond, and over here, I've always heard DSMs say LX (ell-ex) for lighting cues. I sometimes say LX34, and sometimes LXQ34, kind of depends on what's going on at the time.

Stuff that's flown is always a Fly cue - Rail isn't a term I've heard used here in that context.

Sound, Spot (I did hear a very oldschool DSM call Limes), AV, FX, Pyro, Stage/Deck/Truck, references to a specific scenic piece or effect, as long as it's agreed in advance and you know what call is being used.

It's an odd one. I'm a UK-based SMA member, but I won't be taking part in any activites related to the day. I definitely get why they are doing it, and I think it's great that they're getting some recognition for who SMs are and what they do, and have got support from Equity and The Stage, but I feel a bit like, what about all the technicians/stage staff/wiggies/wardrobe/etc etc etc ?

The Green Room / Sleeping in your theatre

I had a stage run for my doubled cast this morning and we open tonight, so I've got a long gap in between shows. I'm going to walk out for some fresh air, but also asked colleagues where was a good place in the building to take a nap.

There's a foam mattress on the fly floor which was suggested, but maybe the green room sofa would be better.

Where do you nap in your theatre ?
