Students and Novice Stage Managers / Paperwork: Help please (Recommend good Forms)

At my high school I am stage managing the production of Rumors by Neil Simon the show in it self is kind of interesting but not that technical a unit set.  This is my second stage management job but I have a huge amount of experience as a technician.  I want to take a professional approach, this is my last production I am a senior at my high school.  What kind of forms can I use to not only look professional but also be professional?  Please help my prompt book is up to par.  I have used recommendations from this web forum to add me.  Please help.
