Blocking notation

I'm looking for a good description of writing down blocking notation. An example would be best. Am putting the finishing touches on a PSM handbook. Will be used by a lot of first time SMs so would love to give them as much information to help them.

I know this is too late for the OP, but just in case somebody else has the same question I've uploaded a sample of how I was taught to take blocking notation. You print out the script one-sided so that each page of text has a blank page accompanying it on which you can write your notations. Hope this helps!

I need help to create a form (mainly design and stiff)i can insert behind my script Or on back(i always photocopy my script to be single sided and off right. Im left handed) that has: mini-ground plan for blocking, sections for blocking notes, acting and director notes,onstage and offstage tech, costume&prop tracking and maybe light/sound/deck cues etc (if that fits). I can either keep it or remove it when calling the show?
It takes too much time to flip to my other tabs for costumes, light, sound , prop information. it messy to keep all my forms beside me in rehearsal, tech and performances.
I personally like using one book to run a show. Carrying two books become messy and can't risk not having the correct info I need.
