I use a form similar to this in my high school program, but wth the excision of a few items (viz., the "grade/section" heading and the "class schedule" box) it could be readily used in amateur, educational, and community theatres.  Excel format.

Dance Audition Sign-In Sheet

This is a standard version of the Dance Audition Sign-In Sheet I created. It was very useful; after everyone signed in - I made a copy for the production staff that was currently auditioning (director, vocal director, choreographer, etc). I communicated with the director beforehand to receive the list of names that needed to be typed. I hope it helps you!  ;D
