I'm trying to figure out for a prompt copy what people to mention for creative and crew? Who is really a part of crew and who is really part of creative? How would you seperate the different jobs in the theatre in those 2 categories?

Hmmmmmm.

You might get varied answers, but I would assume "creative" would be the following:
Director (assistant director)
All Designers
Composer
Fight Director
Choreographer
Stage Management (That is up for debate I am sure)

Crew would be:
ALL RUN CREW
Light Board Operator
Spot Ops
Sound Board Op
Sound/Mic Runner
Wardrobe Crew
Props Run Crew
Fly Crew
Scenic Run Crew

Mostly I agree with Matthew.

Where do you consider scenic artists and props masters?  They're both design and executive in many cases.  And what about Tech Directors and Master Elecs? It will certainly vary from shop to shop and from show to show.

There's also the company leaders like the Artistic Director, General Manager, Production Manager - these are usually grouped in a separate heading from everyone else as facility staff but in many cases the Art. Dir and the PM are making many creative decisions contributing to the final product.

In order to avoid hurt feelings, I sometimes list "Production Staff" and "Crew" instead of "Creatives".  The only people I put under crew in these cases would be run crew, wardrobe crew, board ops.

This is exactly what I do. Anyone involved on the production team as a director, designer, technician, etc are listed as Production Staff. It's pretty all encompassing.  This could also include production managers, producers, box office manager, assistants..whoever else is part of the process.

Crew is, as mentioned, the running crew, board ops, etc.

Wow this is great thanks. So instead as production crew. Who needs to be mentioned in your list of roles and your contact list?

Yeah I just separate it by Cast and Production Team.  Everyone that doesn't perform on stage falls under Production.

The contact list is about you (and anyone else you are going to give it to) having all the info you need to hand, wherever you are, whenever you need it. So list all the people you might ever want to contact! If you are giving it cast and visiting creatives it might be useful to include a few local numbers like the office (incl fax & address), box office and taxi option. also any hotels anyone is staying at (airline flight arrival hotline if working a festival with arriving artists etc).
If more than 2 pages consider putting it on an A3 folded page as almost a mini brochure (opera company size and the like)
Can be helpful to list people's roles or titles under their names for those less familiar with the cast/ visiting creatives.

Thanks! Well I'm only making a mock prompt copy and he's not expecting much from us as we've never done one before.
We're just doing it to get an idea of it!

I break up anyone for stage crew - onstage and spots and deck manager and then production staff (director, designers, td, sm and pm)....
