FORMS: Does anyone have a template for wallet cards?

Could you post it in forms section if you do??

Also, any suggestion as to what should be on a wallet card.. I've seen so many variations.. would be great!

I tried to upload my wallet cards as an example, but they are in MS Publisher and SMNetwork does not allow that file type.  :( If you want to check out another example, you can download them from my site at:

I put all stage management's cell phone #s (provided they've given their okay) the theatre's box office number and main line, and emergency line if they have one, also I'll put the directors cell phone # if they want it on there.

Stage managing is getting to do everything your mom told you not to do - read in the dark, sit too close to the TV, and play with the light switches!

In addition to BalletPSM's list, I like to issue wallet cards with rehearsal studio/theater address, producing office address and a note - "Please call Stage Management at XYZ number if you are running late!"

If you have a rehearsal hotline, I would also list it on there as well.

On the front of my cards, I usually put the theatre's address, phone number, box office number, fax number, SM numbers, CM number, and rehearsal hotline.  On the back I put the performance schedule (though this can be a bit tricky if it's a long run). 

In addition to wallet cards for the cast, I like to do an SM only wallet card that has the phone numbers of the entire cast and crew on it.  It's much handier than carrying the contact sheet around with me at all times.

I too do quicklist of contacts on one side and schedule on reverse.
