I'm a recent California transplant, and have run into the same problem. (Which, since it's a new national privacy law, I'm not sure why I wasn't aware of it in Illinois, Florida or Texas, but, whatever.)  From what I've been able to determine after reading countless HIPPA official websites, primarily, the HIPPA regulations only apply to medical facilities and institutions, but just to protect myself, I've added this to the top of the sheet:

Due to current HIPAA regulations, please be aware of  the following:
•  All information will be kept strictly confidential, in the possession of the Stage Manager.  This information will be destroyed at the end of the term of your contract to protect your privacy.  
•This information will only be used in case of a medical emergency
•This form can NOT be used as a medical consent form, OR without your consent.
•All information that you provide is strictly on a voluntary basis.

I've been in situations where an actor's emergency contact wasn't available, the actor was unconscious, and the only person who had the actor's medical info was their agent.  Since then, I've always asked for the info.  It's always good to have their contact info available.  I don't ask for permission on the sheet, since permission is implied if 911 ever needs to be called.  

There's my 2 cents for the day. :D

The Hardline / Favorite new rule

Here's a fun one -
I just found a new rule in the SPT rulebook - the SM isn't allowed to either lock or unlock the building.  It wasn't there in the last version of the rulebook.
Considering I'm usually the first person in and the last person out... this is a bit silly.  It makes some other poor sap have to show up just to lock and unlock the door.  I can understand it from a liability standpoint, or so that the SM doesn't HAVE to be there to open the building.  But otherwise... I'd much rather have someone just ASK if i'm willing to open a door.  It's not as if we aren't responsible.  Come on!

I love it when new rules show up.  

As for the chicken suit... I hate it when the feathers tickle your nose.  

I've had to dress in a french maid costume (don't ask), a bad 80's outfit complete with a giant ratted side ponytail (which gets caught in your headset, BTW), and wore antlers over my headset for student matinees of Christmas Carol (hey whatever gets you through those 10am calls with tons of kids - the antlers make the kids think you're glad to be there that early in the morning).  No feathers.... Yet.

I asked around to other SMs and actors in town if any of them had an accountant.  Turns out there's a great one (here in Chicago at any rate) who does the taxes for most of the artists in town.  He's got this great worksheet that tells you the categories and what you can deduct each year.  I then set up my files accordingly, and drop reciepts into each file folder throughout the year.  I'll send Kay a copy of this year's in-town expense deduction category list to post.  It's worth every penny to have him do my taxes every year.  He's found me more deductions and saved me thousands of dollars.... it really helped me to sort out what is important to keep and what wasnt.[/img]
